Learning with Experts (LWE) and Polyatrics Limited have secured £79,822 from Innovate UK to develop a groundbreaking digital therapeutic platform, Wellness Interactive Support Hub (W:ISH), focused on social prescribing and the management of long-term health conditions. 

Introduction to W:ISH

W:ISH is designed to connect NHS patients with local services, healthy activities, and support groups through social prescribing. The collaboration between Learning with Experts and Polyatrics aims to integrate personalised healthcare education with digital tools to prevent long-term conditions like diabetes and cardiovascular diseases.

Goal

The project will use human-centred design principles to improve the usability and effectiveness of W:ISH transforming the platform into a fully interactive, social online learning environment leveraging motivational tools to drive long-term engagement and improve patient outcomes.
